Nortriptyline is a tricyclic antidepressant (TCA) used in major depressive disorder. It belongs to the older generation of antidepressants and has a long record of use in clinical practice. Chemically it is a secondary amine, a detail that matters for how well it is tolerated.

Key points

  • Nortriptyline sits in the antidepressant class of tricyclics and is used for major depressive disorder.
  • It is a secondary amine that inhibits noradrenaline reuptake.
  • A dose stays active in the body for around 24 hours.

What nortriptyline is used for

Its main use is major depressive disorder, where it is given to ease low mood and the loss of energy that goes with it. The medicine works through noradrenaline signalling rather than the routes taken by newer antidepressants. A doctor decides whether it fits a particular person after looking at their history and other medicines.

How it works

Nortriptyline blocks the reuptake of noradrenaline, so more of this chemical messenger remains available at nerve junctions. This balanced reuptake inhibition is understood to lift mood and improve energy levels. As a secondary amine tricyclic, it has an improved tolerance profile compared with other TCAs. Benefit builds over two to four weeks.

Safety

Nortriptyline must not be used after a recent myocardial infarction, or by anyone with arrhythmia, mania or severe liver impairment. It can cause drowsiness, and elderly patients need their pulse monitored. Dry mouth, sedation, constipation and dizziness are common at first; blurred vision, urinary retention, tachycardia and weight gain can also occur. Arrhythmia, suicidal ideation and convulsions each need immediate medical attention. Alcohol, MAO inhibitors and cimetidine interact with it. It is not recommended for children. In pregnancy it is used only if clearly indicated, and breastfeeding calls for caution.
